The Role of District Nurse Liaison

The role of the District NurseLiaison nurse is to facilitate the smooth transfer of patients from the hospital to home.

They identify patients in hospital who will continue to require nursing care at home after discharge and liaise between the hospital nurses and community nurses to ensure continuity of care. This is achieved by regular contact with the wards and departments of the hospital, attendance at ward multidisciplinary team meetings and arranging case conferences that draw together hospital and community nurses to discuss individual cases. They identify and make arrangements for nursing equipment that may be required to facilitate care at home

District Nurse Liaison nurses also provide advice and guidance to patients carers and hospital staff on services available from the Cardiff Community Trust such as. They provide ward staff with up to date information on primary care issues.
